When it comes to enrolling your pet in the Parasite Prevention Program we like to make things as simple and convenient for you as possible.
That's why we have multiple ways for you to enrol your pets!
Option 1:
Simply complete our online enrolment form!
Option 2:
Drop into one of our partner veterinary practices in person and one of their helpful staff can sign your pet's up and answer any of your questions. Staff at our partner practices are have received training so they know all about the Parasite Prevention Program.

Option 3:
Call us on 1300 838 627 and we can enrol your pet over the phone. It only takes a few minutes!
Option 4:
Download the necessary forms, complete them and :
- fax to (03) 9923 6522 or;
- mail them to Reply Paid 7332, Karingal Centre Post Office, Frankston, VIC, 3199 (No postage stamp is required) or;
- scan them and email them to us at enrolments@parasitepreventionprogram.com.au
- drop them off at one of our Veterinary Partner Practices
Please complete an enrolment form for each of your pets that you wish to enrol.

You will also need to complete and send us a Direct Debit Request/Credit Card Authority form. Only one form is required even if you are signing up multiple animals.
